Revascularization requests are unique in the way they need to be ordered and therefore we want to provide some tips about how to go about submitting these:
We recommend submitting requests for revascularization CPT codes only after an angiogram has been completed.
If these procedures are completed at the same time, you will need to submit a retro authorization for revascularization requests and include the angiogram report documentation in this request.
Angiogram should demonstrate ≥75% stenosis.
