Levels of injection
Use both the procedure codes and clinical assessment questions to indicate the levels of injection. Utilize the appropriate CPT code to indicate one or multiple levels.
For RFAs: use procedure code 64635 to indicate one level and procedure code 63636 for an additional level.

*Per Medicare CMS guidelines, only 2 levels of injections may be performed at one visit (these can be bilateral injections).
