Determination Letters in the Portal
How to view, download and/or print the final determination letter from the portal:
Navigate to the patient summary. You can get to the patient summary from the dashboard in two ways:
From the dashboard, locate the correct authorization and click the request. Find more information on searching for authorizations here.
From the dashboard, click the "start auth request" button to use the patient search function. Once the correct patient is returned you will see a "patient summary" option.
Once on the patient summary, you will be able to view a list of authorizations for that patient.
On the top right corner of each authorization, click the three dots (⋮).
Select “Print” from the dropdown menu. Then you’ll be able to view and print the Final Determination Letter.
In some cases, you may see additional options when you click the 3 dots. Regardless of what appears, you must always select “Print” to access determination letters. Once on the print page, any available letters will appear in a menu on the left side.
Please note: You may also see a print icon appear within the status bar of the authorization. Clicking this print icon will direct you only to the service summary.
Important Notes:
Members will continue to receive physical determination letters by mail unless a phone call is required by regulations
For providers that are opted in to electronic notices, they will only receive letters when required by regulations
In some phone or fax email requests, either the requestor does not have an account in our system or we aren’t able to map them to an account. In these cases we don’t know the requestor’s provider organization, and the authorization is considered ineligible for electronic notification

Go Paperless: Receive Your Determination Letters Electronically
Cohere Health has made it our mission to reduce waste and transition to electronic final determination letters as the default for all authorizations submitted through our portal. This shift helps reduce delays, improve provider efficiency, and minimize environmental impact.
We’re encouraging all organizations to formally opt in to paperless by reviewing and updating your paperless settings in the portal.
Important note: only an admin user can view and update this preference, and the selection will apply to all users within the organization (see instructions below).
What Does “Paperless” Mean?
If you choose paperless, your organization will no longer receive mailed determination letters. Instead, your team will receive secure email notifications notifying you a decision has been made and prompting you to log into the portal to review that decision. You can view, download, and print letters directly within the Cohere platform (see instructions below).
How to confirm your organization's paperless setting:
Reminder: only an admin user for your organization can view and update this preference.
Log into the Cohere portal
Select ‘My Account’ in the top right of the dashboard
Click ‘Paperless Notification Settings’
Toggle the bar in the pop up message to ‘On’ to go paperless and only receive electronic determinations letters
If the setting is toggled to “On” that means you will only receive electronic determinations letters
If the setting is toggled to “Off” that means you will be mailed paper determination letters and have the ability to access the letters electronically
